MOUNTAIN COMMUNITIES HEALTHCARE DISTRICT IS HELPING TO MAKE SURE YOU ARE COVERED
Mountain Communities Healthcare District (MCHD), which includes Trinity Hospital, Trinity Community Health Clinic and Hayfork Community Health Clinic, is contracted with more health insurance companies than ever. Please see the below list. We are continually adding new contracted insurance companies.
If you don’t see your insurance company on the following list, please contact Kelly Simpson, MCHD’s Director of Revenue Cycle at 530-623-5541, extension 3258, with your phone number and your insurance company. We will attempt to contract with any insurance company plan held by our local community and business members.

In addition, because our Emergency Room (ER) physicians are now directly contracted with our facility, we ensure that patients will not receive any out-of-network ER physician bills for the above contracted insurance companies.
