Notice of Financial Assistance and Charity Care Program
Mountain Communities Healthcare District’s (MCHD) mission is to provide compassionate, high-quality, affordable healthcare services and to advocate for those who are unable to pay for medically necessary services. As such, we continually strive to ensure that the financial capacity of people who need healthcare services does not prevent them from seeking and receiving medically necessary care.
If you don’t have health insurance, or if you have high cost medical bills, our financial counseling staff is here to help you determine eligibility for charity care, discount programs, payment plans, and government programs, such as Medi-Cal and Covered California. We can request a determination for Medi-Cal on the same day as your visit. If you have a pending application for a health coverage program this will not preclude you from being eligible for either program.
Our Financial Counselors are available to work with you to complete any application processes or to answer any questions. If our Financial Counselors are temporarily unavailable, you may also contact our Admitting staff or our Business Services staff for answers to your questions or to get applications for financial assistance or Medi-Cal/Covered California.
Help Paying Your Bill
There are free consumer advocacy organizations that will help you understand the billing and payment process. You may call the Health Consumer Alliance at 888-804-3536 or go to https://healthconsumer.org for more information.
Hospital Bill Complaint Program
The Hospital Bill Complaint Program is a state program, which reviews hospital decisions about whether you qualify for help paying your hospital bill. If you believe you were wrongly denied financial assistance, you may file a complaint with the Hospital Bill Complaint Program. Go to HospitalBillComplaintProgram.hcai.ca.gov for more information and to file a complaint.

Plain Language Summary for the MCHD Hospital Financial Assistance and Charity Care Policy
The purpose of MCHD’s Hospital Financial Assistance and Charity Care Policy is to ensure that our patients have access to affordable and/or free healthcare.
MCHD offers free services to private pay patients with a family size and income level at or below 100% of the Federal Poverty Level, based on the most current Federal Poverty Guidelines.
MCHD also offers financial assistance to patients with a family size and income level between 101% and 400% of the Federal Poverty Level, and that also have medical bills at or above 10% of their annual income.
You should send your application as soon as possible. The application must be submitted to the hospital within 240 days from the time the hospital gives you your first statement.
Financial Assistance is granted only for medically necessary evidenced-based care or for urgent or emergent services.
It is sometimes necessary for this hospital to send out diagnostic tests that we do not perform at our hospital. It is also, sometimes necessary to consult with specialists. If you receive a bill for those outside services, you must contact that organization to ask about their financial assistance and charity care policies.
How to Apply:
To apply for Financial Assistance or Charity Care you would just need to complete a Financial Assistance Application and include a copy of your most recent Federal tax return or your last 3 months of pay stubs. Once you have completed the application you may bring it to the Financial Counselor’s office, which is located through the emergency room doors at the hospital at 60 Easter Avenue, Weaverville, CA 96093 or you may mail it back in to:
